The Practice Health Atlas (PHA) is a decision support and practice planning tool that aims to inspire general practice teams to reflect on their activities and to develop innovative business models for more effective health care services/outcomes. 
The PHA for your practice can be utilised as an information resource to compare with your Practice’s existing knowledge and for business and clinical systems modelling.
There are two components to the PHA:

  • Epidemiological view of practice data within a geographic information context and informed by practice data, national census data and other data sources
  • Clinical and Business Systems Modelling to link the health characteristics of a practice patient population with business opportunities to provide improved health outcomes
